  Orientation Hours: 40 hours Job Summary: We are seeking a skilled and experienced Case Manager to join our interdisciplinary care team in an acute hospital setting. This role is responsible for care coordination, discharge planning, and utilization review, ensuring patients receive appropriate and cost-effective care. The ideal candidate has strong knowledge of Medicare/Medi-Cal guidelines, experience using InterQual criteria, and a proven ability to manage complex discharge needs independently. Key Responsibilities:
  • Perform comprehensive assessments to identify patient needs for post-acute care, DME, home health, and support services.
  • Coordinate and implement safe and timely discharge plans for a diverse patient population including neonates, pediatrics, adults, and geriatrics.
  • Utilize InterQual criteria to determine medical necessity for admissions and continued stay.
  • Perform utilization reviews and collaborate with payers for authorization and medical necessity.
  • Provide patient/family education and facilitate family meetings as part of the care plan development.
  • Collaborate with interdisciplinary teams including physicians, social workers, therapists, and outside agencies.
  • Navigate discharge barriers including homelessness, substance abuse, protective services, and behavioral health crises.
  • Document in Epic electronic medical record system. Requirements:
    • Minimum 2 years of acute care case management/discharge planning experience.
    • Prior travel experience required (first-time travelers may be considered).
    • Strong experience and proficiency with InterQual criteria.
    • Demonstrated competency in arranging SNF placement, ambulance transport, and ordering DME independently.
    • Experience in HMO, IPA, Medicare, Medi-Cal systems.
    • Familiarity with CMS regulations, DRG, HIPAA, and worker's compensation guidelines.
    • Must be able to manage caseloads up to 25 patients ; expected patient ratio is 1:20 during the day.
