Job Opportunity – Hospital Case Manager – California Start Date: ASAP Location: Mission Hills, California Compensation: Details provided to qualified applicants Schedule: Full-time schedule with weekend rotation to meet department needs; 1:20 patient ratio Highlights: Acute care facility setting focusing on care coordination and discharge planning across diverse patient populations including adolescents, adults, and geriatrics Utilize Epic charting system for computerized documentation Work with interdisciplinary support teams including social services and transportation services 40 hours of orientation provided to ensure successful integration Professional dress code: scrubs or business attire accepted No floating required outside assigned unit Requirements: Active California RN license (pending licenses accepted but must be active by start date) Minimum 2 years of acute care hospital Case Manager experience performing care coordination and discharge planning Proficiency with Epic EMR system required Experience utilizing InterQual Criteria and DRG (Diagnosis Related Groups) Demonstrated competency in arranging DME, SNF placement, and ambulance transport independently Knowledge of HMOs, IPAs, and CMS guidelines Proficient in Microsoft applications Strong organizational skills with ability to coordinate and participate in family meetings Experience working with diverse patient populations including those experiencing homelessness, mental health needs, substance use issues, advance directives, POLST, and protective services processes Why It's a Great Opportunity: This position offers the chance to make a meaningful impact on patient outcomes through comprehensive care coordination in an acute care setting.
