Job Description:
A company is looking for a Telephonic Care Navigator - Remote in
CT.
Key Responsibilities
Completes telephonic outreach to DSNP/CSNP members and assists with
care gaps
Educates members on healthcare access and connects them to
community resources
Maintains schedule adherence and ensures compliance with HIPAA
regulations
Required Qualifications
High School Diploma / GED or equivalent work experience
1+ years of call center and/or telephonic customer service
experience
1+ years of professional experience in an office setting using
telephone and computer
1+ years of experience in healthcare, insurance, or community
outreach
Ability to work from home with high-speed internet access
